Parent Survey

As a Youth Ministry, we recognize that no one knows the youth better than their parents. That’s where you come in! Let us know below who your students are, what they are like, and how we can best come alongside you in equipping them for a life of joyful, faithful ministry to Christ.
  • To the best of your knowledge, is your child a follower of Jesus? Please let us know a brief testimony.
  • Please let us know about your family. What are some ways we can encourage and equip your child in their role as a son or daughter, brother or sister?
  • Where does your child attend school? What are some opportunities or challenges your child may have that we should be aware of?
  • What does your child enjoy doing? How are they at developing friendships? What hobbies or activities are they drawn to?
  • Is your child more hands on? Are visual aids helpful? Would they rather read a book? How can we best communicate God's truth to them?
  • What hopes do you have for your child this year? How are you praying for them?
  • Are there any particular struggles or concerns that you have for your child? How can we lovingly support you as a family?
  • Is there anything else that you would like to say?
