18 years old and younger = MINOR – Medical Release Form
19 years old and older = ADULT – Medical Release Form

Medical Release Form – Important Information:

• Forms are due Wednesday, October 17th.
• Do not sign the last page until in front of a notary.
• 18 year olds and younger must have a parent complete the form.
• 19 year olds and up must complete the form and get it notarized. There are several notaries in the church office who can help.
  • Office hours are Monday through Friday 8am – 5pm
